- Failed or missing penetration seals Pipes, cables, trays and ducts crossing fire-rated walls and floors with no tested seal, or a seal that does not match a tested system.
- Damaged or missing fire and cavity barriers Barriers displaced above ceilings, punctured by later services, or never fitted where the void runs past the compartment line.
- Breached compartment walls and floors Openings, linear gaps and head-of-wall joints re-formed and sealed so the line reads continuous again.
- Undocumented previous work Seals with no record behind them, re-validated or replaced, then evidenced so the building can prove what it holds.

The worst things I see all the time when it comes to fire-stopping is pink foam. Like, people love spraying, expanding pink foam into any gap and then thinking that it's fire-stopping. Like, it is not. If anyone tells you that it is, they are wrong. Another really common one is no tags. Proper fire-stopping installation should have tags. It should be real clear, like, you know, it's got the company's name, who installed it, the date it was installed, all of that information. If you don't have that, then you can't guarantee that you've got fire-stopping. Another really common one is just random mastic or insulation stuffed into gaps and calling it fire-stopping. It's not fire-stopping. And one more one is this. When it comes to fire doors, just because it's got FD30 or FD60 on it or a sign that says fire door doesn't make it a fire door. Fire door or fire-stopping is a system and there are loads of things that make it correct. If one of those things is out of place, the system's incomplete. When does a building need remedial firestopping?
Usually after something surfaces the defects: a compartmentation survey, a fire risk assessment, an insurer or client audit, or breaches created by other trades during refurbishment. The trigger is rarely a search for the work itself. It is a finding that now has to be closed out with evidence. Do we have to use the contractor who did the survey?
